Job summary

Aventura Health Group in Austintown, OH is hiring a Full-Time Occupational Therapist (OT) to join our rehabilitation team in a Skilled Nursing and Long-Term Care setting. New graduates are encouraged to apply, with opportunities to grow alongside a supportive interdisciplinary team.

You'll evaluate residents, develop individualized OT plans, address ADLs and mobility, and document progress using Net Health.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Ohio OT license or eligibility for Ohio licensure.
• New graduates are welcome to apply.
• SNF/LTC experience is a plus.
• Familiarity with Net Health and PointClickCare (PCC) is a plus.

Responsibilities

• Evaluate residents and develop individualized occupational therapy plans of care.
• Provide skilled OT interventions addressing ADLs, functional mobility, strength, safety, and independence.
• Establish appropriate goals and monitor resident progress.
• Complete accurate and timely documentation using Net Health.
• Collaborate with nursing, therapy, physicians, and other members of the interdisciplinary team.
• Provide supervision and clinical direction to COTAs as appropriate.
• Educate residents, families, and caregivers regarding treatment plans and recommendations.
• Participate in discharge planning and promote safe transitions; maintain compliance with applicable standards.

What You'll Do

As an Occupational Therapist, you'll help residents maximize their independence, safety, and quality of life through individualized, resident-centered care.

Responsibilities include:

Qualifications

• Graduate of an accredited Occupational Therapy program
• Current Ohio Occupational Therapist license or eligibility for Ohio licensure
• New graduates are welcome to apply
• SNF/LTC experience is a plus, but not required
• Familiarity with Net Health and PointClickCare (PCC) is a plus
• Strong communication, clinical, and organizational skills
• Passion for helping residents achieve their highest level of independence
