Job summary

South Creek Post Acute in Centralia, WA is seeking an Occupational Therapist for a PRN role. The OT must hold an active Washington license and can join a collaborative rehab team focused on ADLs, strength, balance, and independence.

The position offers flexible PRN scheduling with competitive pay of $48-$62/hour depending on experience. Training, documentation in Net Health and PointClickCare, and patient-centered care are emphasized.

Qualifications

• Degree in Occupational Therapy from an accredited program (master's or doctorate preferred).
• Active Washington OT license in good standing.
• CPR certification if required by licensure.

Responsibilities

• Evaluate residents' OT needs, functional limitations, and rehab potential under physician orders.
• Build and update individualized treatment plans focused on ADLs, strength, balance, endurance, and independence.
• Supervise COTAs, aides, and students according to the Washington practice act.
• Collaborate with physicians, nursing, IDT, residents, and families on goals, progress, and discharge planning.
• Participate in care conferences, utilization review, and rehab meetings.
• Coordinate durable medical equipment and adaptive devices as needed.
• Teach home exercise programs and safety techniques to residents and families.
• Train restorative aides and lead staff in-services as needed.
• Complete timely, accurate documentation in Net Health and PointClickCare.
• Follow infection control and safety practices and report equipment issues.
• Contribute to QAPI and rehab program development.

What you'll do

• Evaluate residents' OT needs, functional limitations, and rehab potential under physician orders
• Build and update individualized treatment plans focused on ADLs, strength, balance, endurance, and independence
• Supervise COTAs, aides, and students according to the Washington practice act
• Work with physicians, nursing, the IDT, residents, and families on goals, progress, and discharge planning
• Take part in care conferences, utilization review, and rehab meetings
• Recommend and coordinate durable medical equipment and adaptive equipment (splints, assistive devices, grab bars, and more)
• Teach residents and families home exercise programs, safety techniques, and fall prevention
• Help train restorative aides and lead staff in-services as needed
• Complete timely, accurate, compliant documentation in Net Health and PointClickCare
• Follow infection control and safety practices, and report any equipment issues
• Contribute to QAPI and rehab program development

What you'll need

• A degree in Occupational Therapy from an accredited program (master's or doctorate preferred)
• An active Washington OT license in good standing
• CPR certification if required by licensure
• Comfort with EMR systems; Net Health and PointClickCare experience is a plus
• Strong communication skills and a resident-centered approach
• SNF or post-acute experience preferred; new grads are welcome to apply
