South Creek Post Acute is seeking a PRN Occupational Therapist to help residents regain independence and return to daily activities in Centralia, WA. You’ll pick up shifts that fit your life and join a collaborative rehab team that values your clinical judgment.

Responsibilities include evaluating OT needs, developing individualized treatment plans focused on ADLs, supervising COTAs and aides, coordinating with physicians and the IDT, and documenting in Net Health and PointClickCare.

Qualifikationen

• Degree in Occupational Therapy from an accredited program (MS or Doctorate preferred).
• Active Washington OT license in good standing.
• CPR certification if required by licensure.
• Comfort with EMR systems; Net Health and PointClickCare experience is a plus.
• SNF or post-acute experience preferred; new grads welcome to apply.

Aufgaben

• Evaluate residents’ OT needs, functional limitations, and rehab potential under physician orders.
• Build and update individualized treatment plans focused on ADLs, strength, balance, endurance and independence.
• Supervise COTAs, aides, and students per Washington practice act.
• Collaborate with physicians, nursing, IDT, residents and families on goals and discharge planning.
• Participate in care conferences, utilization review, and rehab meetings.

Ausbildung

OT degree (MS or DPT) from an accredited program

Active Washington OT license
