Hope At Home Health Care is seeking a compassionate Occupational Therapist to join our home health team in Michigan. The OT will assess patients in their homes, develop treatment plans, and supervise assistants as part of a comprehensive care team.

The role emphasizes personalized patient education, collaboration with physicians, and discharge planning. Part-time schedule with visits at patient homes and potential for weekend coverage.

Qualifications

• Occupational Therapy License (Required).
• Licensed to practice as an Occupational Therapist within the state.
• Two years of appropriate experience as an Occupational Therapist. Community/home health experience.
• Demonstrates good verbal and written communication and organization skills.
• Possesses and maintains current CPR certification.

Responsibilities

• Provides Occupational Therapy services to patients according to the physician's plan of care.
• Assesses muscle strength, mobility, ROM, and rehab potential; contributes to the plan of care.
• Directs OT treatment and trains patients and caregivers in use of therapeutic appliances.
• Identifies needs for other home health services and refers as necessary.
• Prepares clinical progress summaries and participates in discharge planning.
• Supervises assistants according to policy and state regulations.

Responsibilities

• Provides Occupationaltherapy services to patients according to a written physician's plan of care. This may include, but will not be limited to: Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment.
  1. Assesses for muscle strength, mobility, ROM—potential for rehab.
  2. Directing Occupationaltherapy treatment.
  3. Instructing patients and families/caregivers in the use and care of therapeutic appliances.
  4. Determining priority needs for Occupationaltherapy.
  5. Reporting to physician patient’s reaction to treatment or changes in condition.
• Initiates Occupationaltherapy program and instructs other personnel and/or family/caregiver members in certain phases of Occupationaltherapy with which they may work with a patient, as well as instructing them as to the goals of the Occupationaltherapy program for the patient by participating in case conferences.
• May train patient in the use of the prosthetic device.
• Identifies patient and family/caregiver needs for other home health services and refers
  as necessary.
• Prepares and submits clinical and progress summaries based on the attainment of goals.
• Participates in discharge planning for patients.
• Prepares and submits a clinical progress summary based on the attainment of goals as directed by Organization policy.
• Provides Occupationaltherapy consultation to home families/caregivers when indicated.
• Provides in-service education programs for nursing organization personnel as needed.
• Participates in the peer consultation process.
• Supervises Occupationaltherapyassistants according to organization policy and state regulations.

Qualifications

• Possesses a degree from a program in Occupationaltherapy approved by an accredited organization.
• Licensed to practice as anOccupationaltherapist within the state.
• Two years of appropriate experience as anOccupationaltherapist.Community/home health experience.
• Demonstrates good verbal and written communication and organization skills.
• Possesses and maintains current CPR certification.
