Job summary

HOPE At Home Health Care in Michigan seeks a compassionate Occupational Therapist to join our home-health team. The ideal candidate holds an OT license, has strong interpersonal skills, and enjoys working directly with clients in their homes.

You will assess patients, develop plans of care, direct therapy, educate patients and families, and coordinate with physicians. This is a part-time role with visits Monday through Friday; compensation is paid per visit at $60–$70.

Qualifications

• OT license required.
• Two years of experience as an OT.
• Current CPR certification.

Responsibilities

• Provides occupational therapy services according to physician plan of care and assesses home environment.
• Directs therapy, instructs patients and families in use of therapeutic appliances.
• Initiates OT program and trains staff and family members.
• Identifies needs for other home health services and refers as necessary.
• Prepares clinical progress summaries and participates in discharge planning.
• Supervises OT assistants as per policy.

Responsibilities

• Provides Occupationaltherapy services to patients according to a written physician's plan of care. This may include, but will not be limited to: Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment.
  1. Assesses for muscle strength, mobility, ROM—potential for rehab.
  2. Directing Occupationaltherapy treatment.
  3. Instructing patients and families/caregivers in the use and care of therapeutic appliances.
  4. Determining priority needs for Occupationaltherapy.
  5. Reporting to physician patient’s reaction to treatment or changes in condition.
• Initiates Occupationaltherapy program and instructs other personnel and/or family/caregiver members in certain phases of Occupationaltherapy with which they may work with a patient, as well as instructing them as to the goals of the Occupationaltherapy program for the patient by participating in case conferences.
• May train patient in the use of the prosthetic device.
• Identifies patient and family/caregiver needs for other home health services and refers
  as necessary.
• Prepares and submits clinical and progress summaries based on the attainment of goals.
• Participates in discharge planning for patients.
• Prepares and submits a clinical progress summary based on the attainment of goals as directed by Organization policy.
• Provides Occupationaltherapy consultation to home families/caregivers when indicated.
• Provides in-service education programs for nursing organization personnel as needed.
• Participates in the peer consultation process.
• Supervises Occupationaltherapyassistants according to organization policy and state regulations.

Qualifications

• Possesses a degree from a program in Occupationaltherapy approved by an accredited organization.
• Licensed to practice as anOccupationaltherapist within the state.
• Two years of appropriate experience as anOccupationaltherapist.Community/home health experience.
• Demonstrates good verbal and written communication and organization skills.
• Possesses and maintains current CPR certification.
