Job summary

HOPE At Home Health Care is seeking a compassionate Certified Occupational Therapy Assistant (COTA) to join our in-home care team on a PRN basis. The ideal candidate will provide skilled OT services in clients' homes, following physician orders and the established plan of care.

You will support patients and families with therapeutic activities, document progress, and collaborate with the OT team to adjust treatment plans as needed.

Qualifications

• Must hold an Occupational Therapy Assistant license in good standing.
• Two years of experience is preferred.

Responsibilities

• Performs skilled procedures as ordered by the physician and according to the plan of care.
• Instructs and guides patients and families in home exercise and daily living activities.
• Prepares and updates medical records and care plans after each patient visit.
• Participates in patient care conferences and care coordination with the OT team.
• Adheres to agency policies and procedures and participates in QAPI activities.

Job description

Qualifications

• Certified Occupational Therapy Assistant (COTA) License Required

Responsibilities

• Understands and adheres to established Skilled Home Health Care Agency policies and procedures.
• Improves or minimizes residual physical disabilities of the patient.
• Returns the patient to optimum and productive level within the patient’s capabilities.
• Participates with all other home care personnel in patient care planning.
• Performs all skilled procedures as ordered by the physician and according to the plan of care established by the OT.
• Consults with OT regarding the change in treatment.
• Instructs patients and family members in-home programs and fine motor movement exercises.
• Participates in in‑service education and presents in‑service programs as assigned.
• Participates in QAPI activities as assigned.
• Attends all patient care conferences as scheduled.
• Prepares medical records and updates care plans for each patient visit in a timely manner as per Agency policy.
• Performs services planned, delegated, and supervised by the OT.
• Assists in preparing clinical and progress notes.
• Participates in educating the patient and family.

QUALIFICATIONS:

• Must meet the requirements for certification as an Occupational Therapy Assistant from a program accredited by the Accreditation Council for Occupational Therapy Education of the American Occupational Therapy Association (AOTO).
• Certification is maintained by the National Board for Certification in Occupational Therapy, Inc. Currently certified in the state(s) in which practicing.
• Is licensed or otherwise regulated, if applicable, as an Occupational Therapy Assistant by the State in which practicing, unless licensure does not apply.
• Two (2) years experience, preferred.
