Join us as an Occupational Therapist at HOPE At Home Health Care

We are seeking a compassionate Occupational Therapist to join our growing team. The ideal candidate will have a can-do attitude, a friendly demeanor, excellent interpersonal skills, and the ability to provide medical support to clients in their homes.

Qualifications

• Occupational Therapy License (Required)

Responsibilities

• Provide Occupational Therapy services according to a physician’s plan of care, including assessment, evaluation, and development of the care plan.
• Evaluate home environment and assess muscle strength, mobility, and range of motion for rehab potential.
• Direct Occupational Therapy treatments and instruct patients and caregivers on the use of therapeutic appliances.
• Prioritize Occupational Therapy needs and report patient reactions or condition changes to physicians.
• Initiate therapy programs, collaborate in case conferences, and train patients or family members as needed.
• Identify additional home health service needs and refer appropriately.
• Prepare and submit clinical and progress summaries, participate in discharge planning, and provide consultation and in-service education.
• Supervise Occupational Therapy Assistants per organizational and state regulations.

Additional Qualifications

• Degree from an accredited Occupational Therapy program.
• Licensed to practice in the state.
• At least two years of relevant experience, including community/home health.
• Good verbal and written communication skills and current CPR certification.
