Job summary

Lucas County Health Center is seeking a full-time care coordination role. You will support providers, nurses, and staff by coordinating patient care across daily schedules and for those not on schedule to ensure quality health outcomes.

You will serve as a key conduit between providers, patients, and medical records, coordinating services, health promotion, and advocacy for workers and the community at large.

Qualifications

• Current Iowa Licensed Registered Nurse, LPN or CMA.
• Previous experience in a medical office.
• ACLS certification.
• Mandatory Abuse Reporter certification within 6 months of employment.
• Ability to read, write, speak, and understand English well.
• Computer literate – Microsoft Office and able to learn additional software programs.

Responsibilities

• Assist the Clinic Team by coordinating care to patients on daily schedules and for those not on schedule.
• Act as primary conduit for transmission of information between providers, patients, and medical records.
• Coordinate services for patients including preventative care, health education, and advocacy.
• Focus on promotion of health, prevention, and protection from hazards; deliver health and safety programs to workers and community groups.
