Job summary

Intermountain Health’s Spanish Fork OB facility seeks an experienced RN for the Maternal Health Connection program. This full-time role supports obstetric patients in diverse settings, including in-person, virtual, and home visits.

You will educate patients, coordinate visits, and collaborate with physicians to implement care plans. Qualified candidates hold a BSN, RN license in Utah, and BLS; CLC and NCC- EF M certification are preferred and may be obtained within stated timelines.

Qualifications

• Current RN license in state of practice.
• BSN degree required; degree verification.
• BLS for healthcare providers required.
• CLC certification or ability to obtain within three months of hire.
• EFM certification from NCC or equivalent preferred.

Responsibilities

• Manage referrals for the Maternal Health Connection program at the program location.
• Coordinate in-person, virtual, and home visits with patients and providers.
• Provide health education and counseling to patients and families.
• Assess patient needs and develop/adjust care plans with the care team.
• Document care and track prenatal and postpartum visits using program templates.
• Coordinate resources and community-based supports as needed.
