Intermountain Health McKay‑Dee Hospital in Ogden, Utah, seeks an experienced Maternal Health Connection RN to support patients in remote or rural areas, enabling obstetrical care within their communities. The RN coordinates visits, conducts health education, and delivers home, virtual, and in‑person services.

The role emphasizes care coordination, patient education, and collaboration with physicians and public health partners.

Qualificações

• RN licensed in practice state; compact licenses must transfer within 60 days of start.
• BSN degree verification is required.
• BLS certification required for healthcare providers.
• Must reside in or near the program location community.
• CLC certification or ability to obtain within 3 months of hire.
• EFM certification from NCC or other verified source.
• Two years RN experience in labor & delivery, obstetrics, or neonatology.
• Reliable transportation for travel to remote clinic sites and homes.
• Proven collaboration and customer-service orientation.
• Experience teaching, presenting, and facilitating learning activities.

Responsabilidades

• Fetal monitoring and biophysical profiles when indicated.
• Lactation counseling and patient education sessions.
• Coordinate visits (in-person, virtual, home visits) with providers.
• Implement care plans and coordinate resources for patients.
• Attend community events to support program goals.
• Collect and track patient data using program note templates.
• Coordinate referrals and manage care across clinics.
