Job summary

Intermountain Health is seeking an experienced Maternal Health Connection RN to support patients in Morgan County and surrounding areas. You will conduct in-person, virtual, and home visits, coordinate care, and educate patients throughout pregnancy and postpartum stages.

The role requires BSN, RN licensure, BLS, EFM, and CLC certification with at least two years in labor & delivery or obstetrics. Travel to remote sites and community settings is required; work is full-time at an offsite Morgan

Qualifications

• Current RN license in state of practice; compact licenses must be transferred within 60 days of start.
• BSN required; degree will be verified.
• Current BLS certification for healthcare providers.
• CLC certification or ability to obtain within 3 months of hire.
• EFM certification from NCC (A-1 or equivalent).
• Two years RN experience in labor & delivery, obstetrics, or neonatology.
• Must reside in or near the Morgan County community.
• Reliable transportation for travel to remote clinic sites and patient homes.

Responsibilities

• Manage referrals for the Maternal Health Connection program at the designated location.
• Utilize nursing process; develop treatment and education plans with physicians.
• Provide health education across in-person, virtual, and home visits.
• Coordinate patient visits and perform health assessments and screenings.
• Plan postpartum care and birthing plans; coordinate related resources and supports.
• Monitor progress relative to pregnancy and postpartum plans.
• Avoid duplication of visits; determine when in-person vs. virtual visits are needed.
