Job summary

Intermountain Healthcare in Utah is seeking a dedicated RN to support the Maternal Health Connection program in Spanish Fork. You will coordinate visits, provide education, and assist with postpartum planning across in-person, virtual, and home visits.

Responsibilities include fetal monitoring, lactation counseling, and collaboration with obstetric care providers to deliver patient-centered maternity care in the community. This role requires travel to remote clinic sites and homes as needed.

Qualifications

• Current license to practice as an RN in state of practice.
• BSN degree required; certification verification.
• BLS certification for healthcare providers.
• CLC certification or ability to obtain within 3 months of hire.
• EFM certification from NCC or equivalent.

Responsibilities

• Manages referrals for the Maternal Health Connection program at the designated location.
• Utilizes nursing process to develop treatment and education plans with physicians.
• Provides in-person, virtual, and home visits for care and education.
• Coordinates patient visits, conducts health assessments and screenings, and plans postpartum care.
• Develops birthing plans and coordinates resource services and supports.
• Monitors patient progress relative to pregnancy and postpartum care plans.
