Job summary

PeaceHealth in Washington is seeking an OBGYN Patient Navigator for a full-time, day shift. The role focuses on improving access to healthcare and social services for patients managing chronic or acute illness, coordinating care with the Care Management team to support stable, coordinated treatment plans.

Qualifications

• Experience in human services or healthcare settings is preferred.
• Bilingual/bicultural is a plus.
• Ability to work with diverse populations and communities.

Responsibilities

• Assist patients and families in understanding resources and coordinate transportation.
• Coach patients on using a medical home and self-management of chronic conditions.
• Develop patient education programs and tools; reinforce education and direct to resources.
• Follow patients through the continuum of care with stakeholders; ensure timely services via referrals.
