Job summary

Thomas Jefferson University Hospitals, Inc. seeks an Obstetrics Nurse Navigator to provide clinical navigation, patient education, and care coordination for obstetric patients across the ambulatory care continuum.

You will serve as a central contact for patients, families, and staff, addressing barriers to care and coordinating referrals, testing, and follow-up to support timely access and improved outcomes.

Qualifications

• Bachelor’s degree in nursing is required.
• Minimum 3 years of nursing experience in ambulatory care, obstetrics, maternal health, or care coordination.
• Pennsylvania RN license is required.

Responsibilities

• Coordinate obstetric patient care across the ambulatory continuum.
• Educate patients and families about diagnoses, treatment plans, and follow-up.
• Identify barriers to care and assist with referrals, scheduling, and authorizations.
• Ensure timely access to appointments, tests, and specialty consultations.
• Collaborate with physicians, nursing staff, and support services to improve outcomes.
