Job summary

Jefferson Health is seeking an Obstetrics Nurse Navigator to provide individualized clinical navigation, patient education, and care coordination for obstetric patients across the ambulatory care continuum.

This role serves as a central point of contact for patients, families, providers, nursing, scheduling, authorization, and ancillary teams to support timely access to care, follow-up, and clear communication.

Qualifications

• Bachelor's Degree in Nursing and active RN license in Pennsylvania.

Responsibilities

• Coordinate barriers to care, referrals, and follow-up with obstetric patients.
• Educate patients and families about diagnoses, treatment plans, procedures, referrals, testing, and follow-up care.
• Ensure timely access to appointments, diagnostic testing, imaging, procedures, referrals, and authorizations.
• Collaborate with physicians, advanced practice providers, nursing staff, scheduling, and ancillary departments to support continuity of care.
• Track patient populations and care gaps to improve outcomes and reduce missed care opportunities.
