St. Luke's University Health Network seeks a Registered Nurse Navigator in Obstetrics to oversee the patient's pregnancy encounter from intake through postpartum, serving as the main contact for care planning questions and education completion.

Qualifications

• Current Registered Nurse license to practice in Pennsylvania required.
• Two years' women's health experience required.
• Ability to coordinate care across pregnancy and postpartum periods.
• Excellent communication and education skills for patients and families.

Responsibilities

• Perform OB intake and maintain pregnancy encounters throughout pregnancy and postpartum.
• Maintain OB patient lists and track trimester progression and transfers.
• Complete required documentation and education related to pregnancy episodes.
• Coordinate care with specialists during pregnancy, anesthesia, and NICU as needed.
• Conduct depression screening during each trimester and postpartum period.
• Review educational classes and resources offered and document flowsheets.
• Refer patients to community resources (e.g., Nurse Family Partnership) where appropriate.
• Demonstrate age-appropriate assessment and communication skills across patient ages.
