Job summary

ProMedica Health and Wellness Center in the United States is seeking an RN Program Coordinator to be the point person for coordinating patient care from discharge through follow-ups. You will manage the patient plan of care, educate on procedures and tests, and review Health Risk Assessments with providers.

You will monitor patient charts for results, review preoperative clearances, triage calls, and offer health advice and referrals as appropriate. This is a full-time, day-shift position.

Qualifications

• Current State license as a Registered Nurse.
• One year of relatable or equivalent experience.
• ACLS and BLS certification.

Responsibilities

• Coordinate care from hospital discharge through follow-ups.
• Educate patients on procedures and testing ordered by providers.
• Review Health Risk Assessments and monitor charts for test results.
• Review preoperative surgical clearance requests with providers.
• Triage patient calls and provide health advice and referrals as appropriate.
