Job summary
Atlantic Health is seeking a Quality Reviewer (CRC) for Home Care in New Jersey. The role conducts timely review of SOC, ROC and Recertification, uses CRC assessment tools to provide feedback on OASIS accuracy and POC, and codes charts based on primary and secondary reasons for home care.
The ideal candidate holds an NJ RN or PT license with at least three years of certified home care experience, understands COPs, and has OASIS/coding certification desirable.
Qualifications
- Current NJ RN or PT License.
- Three Years of Certified Home Care experience.
- Thorough knowledge of home care regulations and COPs.
- OASIS and Coding certification desirable.
Responsibilities
- Provides timely review of SOC, ROC and Recertification documentation using CRC assessment tools.
- Codes chart based on primary and secondary reasons for home care; reviews and advances 485 promptly.
- Uses home care information system and decision support software proficiently.
- Recommends resources to match patient needs for Medicare clients.
- Provides feedback and education to clinical staff regarding OASIS and documentation deficits; notifies managers of non-compliance.
- Educates clinical and intake staff; develops and delivers educational sessions.
