Job summary

Platinum Home Health Care is seeking an experienced Home Care Marketer to develop referral relationships, generate new patient cases, and expand our presence across the community. This field-based role focuses on building connections with hospitals, physicians, social workers, and other referral sources in NYC.

The ideal candidate has at least 1 year of home care marketing experience, deep knowledge of the NYC market, and a track record of generating new referrals.

Qualifications

• Minimum of 1 year of direct home care marketing experience.
• Ability to travel throughout NYC as needed.
• Strong communication and relationship-building skills.

Responsibilities

• Develop and maintain relationships with hospitals, physicians, social workers, case managers, rehabilitation centers, senior centers, community organizations, insurance representatives, and other potential referral sources.
• Generate qualified patient referrals and new home care cases to meet established monthly performance expectations.
• Identify and develop new referral sources and business opportunities throughout assigned territories.
• Conduct regular field visits and maintain consistent communication with existing and prospective referral partners.
• Educate potential patients, families, and community partners about available home care services and assist in connecting eligible individuals with the appropriate internal team.
• Follow up consistently on prospective referrals to support successful conversion into active cases.
• Represent the agency at community events, healthcare facilities, networking opportunities, and other outreach activities.
• Maintain accurate records of marketing activities, referrals, contacts, and follow-up efforts.
• Collaborate with internal departments to facilitate a smooth referral and onboarding process.
• Track progress toward established referral and case-generation goals and provide regular activity updates.
