Responsibilities

• Provide nutrition evaluation and counseling for patients of all ages.
• Address nutrition-related conditions (hypertension, hyperlipidemia, prenatal/postpartum nutrition).
• Develop and implement individualized nutrition care plans.
• Assess nutritional status and recommendations considering social determinants of health.
• Support nutritional supplementation and monitor patient progress.
• Collaborate with clinicians, case managers, nurses and behavioral health therapists.

On-site services include laboratory, pharmacy and language interpretation through interactive video and live interpreters.

Greater Philadelphia Health Action, Inc. (GPHA) is presently seeking:

• Collaborative member of a multidisciplinary healthcare team comprised of primary care clinicians (Pediatric, Internal/Adult Medicine, Family Medicine) and specialists (Clinical Pharmacy/Endocrinology – Infectious Disease – Rheumatology – Podiatry). The healthcare team also includes medical case managers, nurses, and behavioral health therapists.
• Nutrition evaluation and counseling, without limit, to patients of all ages, including but not limited to:
• Hypertension, hyperlipidemia
• Prenatal and postpartum nutrition
• Abnormal weight loss/failure to thrive
• Nutritional supplementation
• Nutritional impact of social determinants of health, medication and behavioral health disorders
