Job summary

Millennium Post Acute Rehabilitation, West Columbia SC, seeks an experienced RN or LPN to lead a high-volume Skilled Nursing unit. This high-impact leadership role reports to the Director of Nursing and drives clinical excellence in a fast-paced environment.

You will oversee admissions/discharges, mentor staff, ensure regulatory compliance, and coordinate care. On-call weekends and nights are required for this leadership position.

Qualifications

• Active RN or LPN license required.
• Experience in Long-Term Care or Skilled Nursing preferred.
• Weekend and night on-call rotation required.
• Strong organizational skills and accountability.

Responsibilities

• Provide clinical oversight of the assigned unit.
• Supervise and mentor nurses and CNAs.
• Coordinate admissions and discharges for high census.
• Conduct chart audits and review clinical systems.
• Participate in IDT and care plan development.
• Lead quality improvement initiatives.
• Manage on-call responsibilities (weekends/nights).
