Job summary

Trinity Health, a faith-based health system, seeks a full-time registered nurse to develop, implement, and evaluate clinical practice guidelines and care paths for patients. You will collaborate with the care team to ensure quality outcomes and serve as a liaison among patients, families, physicians, and staff to coordinate care across settings.

Qualifications

• Graduate of an accredited nursing program required.
• BSN preferred.
• Current/valid RN license required and BLS/ACLS as applicable.

Responsibilities

• Develop, implement, and evaluate clinical practice guidelines/care paths for patients.
• Collaborate with the health care team to ensure quality patient care and identify care problems.
• Act as liaison with patients, families, physicians, and staff to coordinate care across settings.
• Welcome calls from patients and families, answer questions, and conduct follow-up as needed.
• Provide education and resources to patients and families to support coping and self-care.
• Review literature and disseminate findings; maintain knowledge base of ongoing clinical research.
