Job summary

RYZE on the Avenue in Chicago, IL seeks an experienced Nursing Supervisor to oversee resident care and staff. You will lead an interdisciplinary team, ensure quality care, and report incidents promptly. Strong leadership and RN licensure in Illinois are required.

You will mentor staff, participate in committees, and maintain high standards of clinical documentation and infection control in a dynamic long-term care setting.

Qualifications

• Graduate of an accredited School of Nursing with RN licensure in Illinois.
• Minimum 5 years in long-term care with at least 2 years in supervision.
• Proficient with facility computer systems and documentation.

Responsibilities

• Oversee resident care during shifts and report incidents promptly.
• Lead interdisciplinary team and participate in committees.
• Ensure high-quality care and monitor patient status changes.
• Perform nursing assessments and communicate changes to physicians/families.
• Complete rounds and monitor dining and environmental standards.
• Collaborate with the nursing director to orient and educate staff.
• Review medical records for accuracy and completeness.
• Monitor infections and report trends to leadership.
• Assist admissions with nursing input and staff orientation.
