The position requires RN licensure in Illinois, with at least five years in long-term care and two years in a supervisory role.

Qualifications

• Graduation from an accredited School of Nursing with 5 years of long-term care nursing experience, including at least 2 years in a supervisory role.
• Must hold Illinois RN license.
• Proficient with facility computer systems.
• Strong English communication skills, both written and spoken.
• Commitment to Resident Rights, confidentiality, EEO and ADA.

Responsibilities

• Assigns and oversees resident care during shifts, ensuring timely reporting of incidents to the Administrator and Director of Nursing.
• Participates in committees/programs as directed by the Director of Nursing.
• Oversees resident care to promote optimal physical, mental, and psychosocial functioning for the unit.
• Performs nursing assessments and communicates significant changes to physicians, families, or responsible parties.
• Conducts rounding and monitors dining room during meals to meet resident needs and environmental standards.
• Collaborates with interdisciplinary team to provide holistic care.
• Reviews clinical records for completeness and accuracy.
• Monitors and reports infections.
• Acts as a resource for nursing staff and reviews admissions with the Director of Nursing and Admissions Director.
• Collaborates on orientation and ongoing education for clinical staff.

Job Summary

We are seeking an experienced Nursing Supervisor to join our team at RYZE on the Avenue in Chicago, Illinois. As a Nursing Supervisor, you will be responsible for overseeing resident care, managing staff, and ensuring the highest level of quality care is provided to our residents. Duties and Responsibilities

• Assume responsibility and accountability for resident care during shift, ensuring complete and prompt reporting of incidents to Administrator and Director of Nursing
• Actively participate in committee/programs as directed by Director of Nursing
• Oversee resident care to promote the highest level of physical, mental, and psychosocial functioning possible for assigned unit
• Perform nursing assessments regarding the health/status of residents and ensure that significant changes in resident condition are communicated to the physician, family, or responsible party
• Complete rounding throughout the facility to ensure resident care needs and environmental standards are met, including monitoring of dining room during meal times
• Work collaboratively with other members of the interdisciplinary care team to provide holistic care
• Review clinical records for completeness and accuracy as necessary
• Monitor, track, evaluate, and report infections
• Act as a resource for nursing staff and review applications for admission to the unit with Director of Nursing and Admissions Director
• Collaborate with Director of Nursing to identify and provide orientation and continuing education for clinical team members

Qualifications

• Graduation from an accredited School of Nursing plus 5 years of experience as a long-term care nurse, with a minimum of 2 years supervisory experience in acute, sub-acute, or long-term care
• Must be licensed as a Registered Nurse in the State of Illinois
• Proficient in the use of computer and programs utilized by the facility
• Ability to communicate in English both orally and in writing
