Job summary

ALIYA Healthcare Consulting seeks a compassionate Registered Nurse (RN) to provide total care for residents in a skilled nursing facility. You will oversee shifts, delegate tasks, and ensure adherence to facility policies while collaborating with the health team to implement care plans.

The role requires an active IL RN license, strong interpersonal skills, and the ability to educate residents and families about care plans.

Qualifications

• Graduation from an accredited nursing education program.
• Active RN license in Illinois.
• No history of abuse convictions.
• Strong interpersonal and communication skills.
• Ability to follow facility policies and care procedures.
• Ability to work with residents of varying ages and medical needs.
• Must meet physical job requirements including lifting, bending, and standing.

Responsibilities

• Provide total care for residents on assigned unit during scheduled shift as an RN.
• Collaborate with allied health team to develop and implement individualized care plans.
• Administer prescribed medications and treatments per policy and evaluate effectiveness.
• Maintain an environment that supports residents' physical, emotional, and social well-being.
• Educate residents and families about care plans, treatments, and procedures.
• Monitor team performance and support staff development, evaluations, and corrective actions as needed.
• Assign and oversee nursing duties to ensure tasks are completed appropriately and efficiently.
• Stay current on facility procedures and participate in in-service and continuing education.
• Identify and respond to significant changes in residents' conditions.
• Accurately document care provided and changes in condition per guidelines.
• Act as a charge nurse and provide clinical support and decision-making guidance.
