Job summary

South Creek is seeking a Director of Nursing Services (DNS) to lead our nursing team in Centralia, WA. You will oversee all nursing operations and resident care, while maintaining top-tier 5-Star quality scores and ensuring survey readiness.

The role requires an active WA RN license, DNS/ADON or nursing leadership experience, and strong MDS knowledge to optimize reimbursement and outcomes. A people-first leadership style is essential for success.

Qualifications

• Active Washington RN license.
• DNS, ADON, or nursing leadership experience in skilled nursing / post-acute care.
• Strong MDS knowledge preferred.
• Solid regulatory and survey experience.
• People-first leadership style that builds trust and accountability.

Responsibilities

• Lead and oversee all nursing operations, staff, and resident care.
• Maintain and build on our 5-Star Quality Measures and survey readiness.
• Drive accurate MDS processes and partner closely with the MDS team on reimbursement and quality.
• Recruit, develop, and retain nursing staff.
• Work alongside the Administrator and IDT as a key member of the leadership team.
