Qualifications

• Specialized Diploma Graduate of an accredited school of nursing.
• 3 years as an RN in an acute care setting.
• 1 year in specialty area.
• Knowledge of patient- and family-centered practice.
• Cultural awareness and respectful care.
• Effective communication and education skills for patients and families.
• Experience with discharge planning and post-discharge care.
• Ability to mentor staff in evidence-based practices.

Responsibilities

• Interacts with co-workers, patients, and families per values of Jefferson.
• Integrates evidence-based practice with clinical expertise and patient preferences.
• Leads education for patients and families and coordinates care.
• Ensures safe care by following nursing protocols and guidelines.
• Uses information systems to communicate and manage knowledge.
• Leads and coordinates care within nursing and inter-professional teams.
