Job summary

californian is seeking an experienced Licensed Vocational Nurse (LVN) to join our Skilled Nursing Facility team in California. The LVN will provide direct resident care under the direction of an RN, administer medications, monitor vitals, and contribute to individualized care plans.

Qualifications

  • Valid and current CA LVN license.
  • Graduate of an accredited nursing program.
  • Minimum of 1 year experience in SNF, acute care, rehab, or behavioral health.

  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong critical thinking and problem-solving skills.

Responsibilities

  • Provide direct nursing care to residents, including administering medications and treatments, monitoring vital signs, and documenting changes.
  • Assist with developing and implementing individualized care plans.
  • Coordinate care with physicians, therapists, and other healthcare professionals.
  • Ensure accurate, complete, up-to-date documentation and charting.
  • Assist with activities of daily living (ADLs) as needed.
  • Educate residents and families about healthcare needs and treatments.
  • Respond to emergencies and provide first aid as needed.
  • Adhere to facility policies, procedures, and regulations.

Job description

We seek an experienced, compassionate, skilled Licensed Vocational Nurse (LVN) to join our team at a Skilled Nursing Facility. As an LVN, you will be responsible for providing direct nursing care to our residents under the direction of a registered nurse (RN) and ensuring that the facility meets all applicable regulations and standards. The AM, PM and NOC shifts are available.

This is a full-time position for the 7am to 3pm, 3pm to 11pm and 11pm to 7am shift.

Responsibilities:

  • Provide direct nursing care to residents, including administering medications and treatments, monitoring vital signs, and assessing and documenting changes in their condition.
  • Assist with developing and implementing individualized care plans based on residents' needs and goals.
  • Coordinate care with other healthcare team members, including physicians, therapists, and other healthcare professionals.
  • Ensure that all documentation and charting are accurate, complete, and up-to-date.
  • Assist with activities of daily living (ADLs) as needed.
  • Educate residents and families about their healthcare needs and treatments.
  • Respond to emergencies and provide first aid as needed.
  • Adhere to all policies, procedures, and regulations governing nursing care in the facility.

Qualifications:

  • Valid and current LVN license in the state of California
  • Graduate of an accredited nursing program
  • Minimum of 1-year experience in a skilled nursing facility, acute care setting, in-patient rehabilitation, or in-patient behavioral health setting
  • Strong clinical skills, including assessment, planning, and implementation of nursing care
