Job summary

Victorian Post Acute in San Francisco, CA is hiring a Licensed Vocational Nurse (LVN/LPN) to join our clinical team. You will provide direct nursing care, administer medications, monitor resident conditions, and support CNAs to ensure high-quality, resident-centered care aligned with physician orders and care plans.

We offer Full Time and Part Time shifts, a competitive hourly pay range of $36.00–$38.00, comprehensive Medical, Dental, and Vision insurance, PTO, a 401(k) plan, and opportunities

Qualifications

• Current, active, and unrestricted California LVN/LPN license.
• Graduate of an accredited LVN/LPN nursing program.
• Long-term care, skilled nursing, or post-acute experience preferred.
• Strong clinical, communication, and leadership skills with a commitment to resident care excellence.

Responsibilities

• Provide direct nursing care and medication administration in accordance with physician orders and resident care plans.
• Supervise and support CNAs to ensure quality resident care and regulatory compliance.
• Monitor resident conditions, document clinical updates, and communicate changes to physicians and families as needed.
• Assist with admissions, discharges, treatments, assessments, and interdisciplinary care coordination.
• Direct daily nursing activities and maintain adherence to facility policies and procedures.
• Maintain accurate resident records, treatment documentation, and physician orders.
• Complete incident reports and participate in quality assurance initiatives.
• Collaborate with nursing leadership to ensure smooth shift operations and resident-centered care.

Available Shifts:

Full Time AM (7:00 AM to 3:30 PM)

Part Time PM (3:00PM to 11:30PM)

Pay Range: $36.00-$38.00 per hour (based on experience)

• Participate in facility surveys, safety drills, and required in-service training programs.

Qualifications
