Job summary

The Californian is seeking an experienced LVN to join our Skilled Nursing Facility team in Pasadena, CA. You will provide direct nursing care under RN supervision, administer medications, monitor residents, and document changes in condition.

We offer a compassionate, collaborative work environment with AM, PM and NOC shifts. Responsibilities include care planning, coordinating with physicians and therapists, educating residents and families, and adhering to all policies and regulations.

Qualifications

• Valid California LVN license
• Graduate of accredited nursing program
• Minimum of 1 year experience in a skilled nursing facility, acute care, inpatient rehab or inpatient behavioral health setting
• Strong clinical, assessment, planning and implementation skills
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Strong critical thinking and problem-solving abilities

Responsibilities

• Provide direct nursing care to residents, including administering medications and treatments, monitoring vital signs, and documenting changes in condition
• Assist with developing and implementing individualized care plans based on residents' needs and goals
• Coordinate care with physicians, therapists, and other healthcare professionals
• Ensure documentation and charting are accurate, complete, and up-to-date
• Assist with activities of daily living (ADLs) as needed
• Educate residents and families about healthcare needs and treatments
• Respond to emergencies and provide first aid as needed
• Adhere to policies, procedures, and regulations governing nursing care in the facility

The Californian -

We seek an experienced, compassionate, skilled Licensed Vocational Nurse (LVN) to join our team at a Skilled Nursing Facility. As an LVN, you will be responsible for providing direct nursing care to our residents under the direction of a registered nurse (RN) and ensuring that the facility meets all applicable regulations and standards. The AM, PM and NOC shifts are available.

This is a full-time position for the 7am to 3pm, 3pm to 11pm and 11pm to 7am shift.
