Job summary

Union Plaza Care Center is seeking an experienced RN with wound care leadership to oversee nursing operations and specialized wound care for residents. This role emphasizes clinical leadership, education, documentation, and collaboration with physicians to improve resident outcomes.

The position requires an active NY RN license and Wound Care certification, with preferred infection control certification and LTC experience.

Qualifications

• Active New York State RN license is required.
• Wound Care Certification is required.
• Infection Control Certification is preferred.
• Minimum of 1 year of nursing home or long-term care experience is preferred.
• Previous nursing leadership or supervisory experience is preferred.

Responsibilities

• Assist the Director of Nursing with planning, coordination, and oversight of daily nursing department operations.
• Assess residents with wounds and those who develop wounds.
• Collaborate with physicians and the interdisciplinary team on wound care plans.
• Lead weekly wound rounds, assessments, treatment recommendations, and documentation.
• Maintain weekly wound tracking reports and report non-healing wounds promptly.
• Track, analyze, and distribute weekly decubitus ulcer statistics.
• Provide in-service education on wound care practices and documentation.
• Prepare quarterly QA reports and participate in quality improvement initiatives.
• Observe dressing changes for proper technique and staff competency.
• Support nursing staff with clinical guidance, education, and resident care needs.
• Educate residents and families on wound care, prevention, and treatment.
• Promote compliance with NYS regulations and facility policies.
• Engage in ongoing wound care education to stay current.
