Job summary

Westchester Disabled On the Move in Yonkers, NY is seeking a Transition Specialist to assist consumers at risk of nursing facility placement by coordinating services and benefits to help them remain or return to the community.

The role involves developing ISP plans, bi-weekly contact with participants, and collaboration with care teams, community providers, and grantor requirements. Requires a degree in human services or related field and strong communication skills.

Qualifications

• Bachelor’s degree in human services or social services field.
• Associate degree with two years of relevant experience.
• Knowledge of Medicaid eligibility and coverage.
• Strong written and oral communication and organizational skills.
• Ability to work independently and as part of a team.
• Must own transportation and have a valid driver’s license.
• Bilingual skills preferred; fluency in a second language is a plus.

Responsibilities

• Identify services needed by consumers at risk of institutionalization.
• Develop Individualized Service Plans (ISP).
• Maintain bi-weekly contact with participants as required by grantor to ensure services needed are provided.
• Provide independent living skills training and advocacy as needed.
• Assist with applications for benefit programs.
• Coordinate with the applicant’s Discharge Planner, Care Manager and/or Service Coordinator and other staff.
• Enter intake data into the grantor’s dashboard and WDOM’s database.
• Promote the program through outreach to Community Based Organizations and healthcare providers.
• Collaborate with WDOM staff to ensure services and support are available to serve the individual.
