Nursing Home Administrator - Transitional Care Unit (Savannah, GA, 31405, US)

Job Summary

The Nursing Home Administrator is responsible for the daily operation of the Transitional Care Unit. This individual will utilize resources effectively and efficiently, to attain and maintain the highest level of care for TCU patients, in accordance with regulatory standards.

Work will be performed primarily indoors at one of our facilities, throughout all areas, including in patient rooms, and on carpeted and/or tiled floors. Work will be performed routinely around other co-workers, healthcare staff, patients, and guests. Due to the nature of facility’s business, worker may be exposed to occasional slippery floors, objects on floors, chemicals, sharp objects, hazardous materials and waste (including human), blood borne pathogens, and communicable diseases, as well as high-stress medical and/or life threatening situations.

Essential Duties and Responsibilities
  • Follows approved organizational and hospital policy and procedures, and exercises ethical and sound leadership decision for the operation of the TCU.
  • Ensures that nursing services are planned, implemented, and evaluated to maximize patient quality of life and quality of care with the integration of patient rights with all aspects of patient care.
  • Works closely with Physicians treating patients on TCU and MDS Coordinator to ensure completion of required documentation to support services provided to patients.
  • Ensures that the unit complies with applicable federal, state, and local standards and regulations and that patient and staff information is made available only according to the state or federal regulations.
  • Coordinates the development and evaluation with the health care team of patient care goals and policies in order to assure that adequate resources, environments, and services are provided to patients, meeting regularly with healthcare team to assure highest practicable care is being delivered.
  • Ensures patient dignity and right to privacy and that patients are free from sexual abuse, physical abuse, mental abuse, corporal punishment, exploitation, neglect and involuntary seclusion.
  • Ensures that personnel are present in number and ability to attain or maintain the highest practicable level of physical, mental, and psychosocial well being for each patient that meets the state and federal regulations.
  • Develops, implements, and evaluates fire, emergency and disaster plans to protect the safety and welfare of patients, staff, and property.
  • Observes, monitors, and evaluates outcomes of TCU specific programs, policies and procedures to ensure effectiveness and to fulfill administrative and professional responsibility.
  • Perform other duties as assigned.
Qualifications
  • Current State Nursing Home Administrator License.
  • A four-year baccalaureate degree from an accredited college, with a minimum of two years of experience in a Transitional/Skilled Nursing Unit setting, and two years of experience in a direct supervisory position.
  • Professional appearance.
  • Effective verbal, written and persuasive communication skills.
  • Organization, time management, and ability to work with others.
  • Demonstrated proficiency with computers, software and a variety of office equipment. Navigation of electronic medical record.
  • Must exercise sound judgment in handling confidential information.
Physical Requirements and Work Environment
  • Moderate physical activity: Push, pull, move, and/or lift a minimum of ten (10) pounds to a minimum height of three (3) feet and be able to push, pull, move, and/or carry such weight a minimum distance of fifty (50) feet.
  • Standing and/or walking for more than four (4) hours per day.
  • Bending and/or stooping for more than one (1) hour at a time.
  • Sitting for more than two (2) hours at a time.
  • Requires consistent computer work with repetitive typing and concentrating on computer.
