Job summary

UPMC Health Plan is seeking a full-time CHC Service Coordinator, Nursing Facilities to support Community HealthChoices in Snyder, Mifflin and Juniata Counties. The role provides service coordination across the care continuum to meet members’ health needs.

The position requires a Bachelor’s degree in social work, psychology, or related field, with practicum or healthcare experience. Travel about 50% to nursing facilities; daylight hours, Monday–Friday; hybrid work arrangement.

Qualifications

• Requires a Bachelor's degree in social work, psychology, or related field with practicum experience preferred OR three (3) years of experience in a social service or healthcare setting.
• Preferred experience with LTSS; ability to coordinate across health, behavioral, and social services.
• Cultural competence and sensitivity to diverse backgrounds.

Responsibilities

• Establish and build strong relationships with internal teams and partners.
• Educate on and coordinate community resources for medical, behavioral, and social services.
• Manage an active caseload based on state ratios and acuity.
• Collect program data to track participant progress and ensure regulatory deadlines.
• Prepare reports and update service plans per governing bodies.
• Ensure assessments are completed within required time frames and inform care options.
• Lead Person-Centered Service Planning (PCSP) process and implement PCSPs.
• Assist members in obtaining LTSS and Medical Assistance services.
