Job summary

UPMC Health Plan is seeking Service Coordinators in Erie, PA to join the Community HealthChoices team. The role involves coordinating care across medical, behavioral, and social services to improve health outcomes for Members.

This is a hybrid position requiring daily community travel and field engagement, with responsibilities including caseload management, data reporting, and assisting Members to access LTSS and Medical Assistance services.

Responsibilities

• Establish and build relationships with internal teams and partners to foster collaboration.
• Educate on and coordinate community resources, including medical, behavioral, and social services.
• Manage an active caseload per state ratios and track progress for OPS reporting.
• Prepare reports and update service plans to meet regulatory timelines.
• Lead the Person-Centered Service Planning (PCSP) process and assist Members with HCBS services.
• Identify and connect Members to LTSS, Medical Assistance, and non-Medicaid supports.
• Inform Members about LTSS and DHS Fair Hearing rights; assist with requests as needed.
• Collect Member information to inform PCSP; reevaluate care levels annually.
• Coordinate with providers to ensure seamless physical and behavioral care; maintain confidentiality and HIPAA compliance.
• Work in the field approximately 75% of the time; ability to operate in a virtual setting.
