Job summary

UPMC Health Plan is seeking Service Coordinators in Monroe Pike, PA to join the Community HealthChoices team. This hybrid role requires daily field visits to support Members in the home and community.

Candidates should have strong case management and care coordination experience, in-home assessments, and the ability to work with adults with complex medical, behavioral, and social needs. Travel and HIPAA compliance are essential.

Qualifications

• Bachelor's degree in social work, psychology, or related field; practicum preferred.
• Three years of social service or healthcare setting experience acceptable.
• Experience with LTSS, seniors, or disabilities preferred.
• Willingness to travel fieldwork ~75% and work virtually.

Responsibilities

• Build strong relationships with team and providers; coordinate resources.
• Manage active caseload per state ratios; track progress for OPS reporting.
• Prepare reports and update service plans; ensure timely assessments.
• Lead PCSPs and assist Members in obtaining HCBS services.
• Identify LTSS and Medical Assistance resources; facilitate access.
• Inform Members about LTSS options and fair hearing rights.
• Collect Member preferences and goals to inform PCSP.
• Coordinate with providers for seamless care and maintain HIPAA confidentiality.
• Travel to the field ~75% and work independently in a virtual setting.
• Must have valid driver's license; reliable transportation; private workspace.
