Job summary

UPMC Health Plan is seeking a full-time CHC Service Coordinator (Nursing Facility) to support Community HealthChoices in Allegheny County. The role is hybrid, with 50% travel to nursing facilities, Monday–Friday daylight hours.

The CHC Service Coordinator provides service coordination across care continuum to improve health outcomes for Members, addressing LTSS needs and ensuring safe transitions between facilities and community care.

Qualifications

Bachelor's degree in social work, psychology, or related field; practicum preferred.
Alternatively, at least 3 years of experience in social services or healthcare.
Cultural competency and sensitivity to diverse backgrounds.

Responsibilities

Establish and build relationships with internal teams and partners.
Educate and coordinate community resources for medical, behavioral, and social services.
Manage an active caseload based on state mandates, residential setting, care level, and acuity.
Collect program data to track participant progress monthly for OPS reporting.
Prepare reports and update service plans per governing bodies.
Ensure assessments are completed within required time frames and assess care options.
Lead the Person-Centered Service Planning (PCSP) process.
Assist members in obtaining LTSS and Medical Assistance services.
Coordinate access to non-Medicaid funded services and supports.
Maintain confidentiality and HIPAA compliance.
