21st Century Home Health Services (21HHS) seeks a Clinical Supervisor - Nursing to lead a hybrid team delivering in-home patient care across the Bay Area. You will coordinate RN Case Managers and LVNs, provide staff development, and ensure timely escalation of clinical issues.

You will oversee field supervisory visits, drive training initiatives, monitor patient outcomes, and maintain strong relationships with customers and clients. A CA RN license, BLS, and leadership experience are required.

Qualifikationen

• Two years' management or supervisory experience within the past five years in a health care setting.
• Graduate of an accredited nursing program and active CA RN license is required; BLS certification.
• Strong leadership and coaching skills; ability to train and mentor staff.

Aufgaben

• Oversee field clinicians delivering care in patients' homes.
• Coordinate training and educational programs for nurses within the field.
• Assess staff learning needs and formulate plans with measurable goals.
• Serve as first point of escalation for field clinicians and patient-care issues.
• Convene bi-weekly Case Conferences with clinical team to review progress and adjust care plans.
• Review patient cases, documentation, and clinical outcomes to improve quality and reduce readmissions.
