Job summary

21st Century Home Health Services is seeking a Clinical Supervisor - Nursing to lead RN Case Managers and LVNs, ensuring coordinated patient care in Marin County. The role blends field supervision with on-site office duties, reporting to the Director of Nursing.

You will train staff, manage case conferences, and monitor clinical outcomes to reduce readmissions and improve patient satisfaction. A CA RN license and BLS are required, with a competitive benefits package and bonus opportunities.

Qualifications

• Two years' management or supervisory experience within the past five years in a health care setting, preferably in Home Health.
• Graduate of an accredited school of nursing, Bachelor’s Degree preferred.
• Current CA Registered Nurse License (RN).
• Current Basic Life Support (BLS) Card.
• Valid CA Drivers License with acceptable driving record.

Responsibilities

• In the field oversight of clinicians who provide care and treatment to patients in their home.
• Coordinate training and educational programs for nurses in the field.
• Assess staff learning needs and set goals for the next evaluation period.
• Serve as first point of contact for field clinicians to assist with patient care escalations.
• Coordinate at least bi-weekly Case Conferences with clinical team to discuss progress and revise care plans.
