21st Century Home Health Services Inc. (21HHS) is seeking a Clinical Supervisor - Therapy to oversee field clinicians delivering in-home patient care.

This leadership role combines field visits with office-based supervision in the San Jose area, ensuring coordinated, high-quality services for patients in residence. The position requires two years of supervisory experience in rehab therapy, a CA license, and an active BLS certification.

Qualifications

• Two years management or supervisory experience in a rehabilitation therapy department or program, preferably home care.
• Current CA Physical Therapist license
• Current Basic Life Support (BLS)
• Valid CA Drivers License with acceptable driving record
• All license and certifications must be current at time of hire and sustained throughout employment

Responsibilities

• Oversee field clinicians providing patient care in patients’ homes.
• Serve as first point of contact for field clinicians to provide timely support.
• Coordinate training and educational programs for nurses in the field.
• Assess staff learning needs and develop plans for the next evaluation period.
• Lead bi-weekly case conferences with clinical team to discuss patient progress.
• Review cases and guide clinicians to improve performance and care delivery.
• Be available during operating hours to assist clinicians as needed.
