The role involves supporting admissions, transfers and discharges, maintaining medical records, collaborating with providers, overseeing audits, and delivering resident care with professionalism.

Qualifications

• Associate minimum, Bachelor’s Degree preferred.
• Area of Study: Nursing.
• Years of Experience: 5-10+ years.
• Type of Experience: Direct resident care experience, previous experience in similar position, experience as a clinical nurse.
• Special Certifications: Must be RN/LPN.
• Language Skills: Acceptable fluency in English to perform duties.
• Technical Competencies: Excellent computer skills.
• Information Systems: Point Click Care.

Responsibilities

• Maintain proper medical records, including PCC documentation.
• Monitor health records, incidents, and follow-ups with residents/families.
• Develop relationships with professional providers of care.
• Assist staff with admissions, transfers, and discharges.
• Monitor falls, infection control, skin issues, and med delivery errors.
• Audit quality parameters and schedule in-services.
• Oversee daily PCC documentation completion and adherence.
• Complete audits as directed by the DON.
• Provide staff in-service and education.
• Communicate with health care providers for orders and updates.
