Job summary

Lutheran Senior Services in Peoria, IL seeks a Licensed Practical Nurse (LPN) to provide and coordinate resident care, administer medications, monitor IV therapy, and support daily living activities to meet physical, psychosocial, and functional needs. You will work with the interdisciplinary team, ensure regulatory compliance, maintain accurate EMR documentation, and mentor caregiving staff.

The role requires active LPN licensure, CPR certification, and willingness to work flexible hours

Qualifications

• Active LPN license in IL.
• CPR certification required.
• Gerontological nursing experience preferred.

Responsibilities

• Provide and coordinate care to residents per licensure.
• Administer medications and monitor treatments.
• Work with interdisciplinary team to develop individualized care plans.
• Maintain accurate EMR documentation and ensure regulatory compliance.
• Oversee and mentor nursing staff and aides as assigned.
• Identify and report changes in resident status to supervisors.
• Ensure safe, clean, and comfortable environment for residents.
• Assist with IV therapy as trained and permitted.
• Follow EverTrue policies and state/federal regulations.
• Participate in quality improvement and staff education.
