Reid Health is seeking an RN for the PACE program on the day shift, 40 hours weekly, 8:00am–5:00pm, to develop and coordinate home care services for PACE participants, including DME and nursing home care.
The role requires ASN or BSN, Indiana RN license, and 1 year of geriatric RN experience (3+ years with supervision preferred). You will assess needs, plan with the IDT, and ensure CMS PACE compliance.

Responsible for the development and implementation of home care services for Reid Health PACE participants, including the coordination of all Durable Medical Equipment and nursing home care.
Overview of Responsibilities
Minimum Education Required: Associate of Science in Nursing (ASN)
Minimum Education Preferred: Bachelor of Science in Nursing (BSN)
Minimum Experience Required: 1 year of experience as RN in geriatric services
Minimum Experience Preferred: 3 or more years of thorough knowledge of current community health nursing practice as a RN in geriatric services with 1 year of experience in supervision
Licensure: Licensed as a RN in the State of Indiana
Certifications: BLS, ACLS, CPR
