Ebenezer Integrated Care & Rehab (EICR) in downtown St. Paul, MN is seeking a Nursing Assistant (CNA/NAR) to provide resident care in Transitional Care, Long Term Care units.

The role features a part-time schedule with 48 hours every two weeks, night shifts, and rotations including every other weekend/holiday. Responsibilities include personal cares, nutrition support, hydration, and accurate documentation in the EHR.

Qualifications

• Nursing Assistant Registration on the MDH Nursing Assistant Registry by start date – OR Ebenezer’s Nursing Assistant Course or Ebenezer’s Pre-CNA program; 4-month post hire to activate on the Registry.
• High School Diploma or GED.
• 1 year of experience preferred.

Responsibilities

• Assist residents with personal cares per individual plan of care including AM/PM cares and weekly baths.
• Provide nourishment, hydration, and monitor intake; document meal consumption.
• Maintain cleanliness and safety in resident rooms and work areas; report equipment issues.
• Document resident cares promptly in the EHR and communicate changes to the nursing team.

Ebenezer Integrated Care & Rehab (EICR) serves seniors with long-term, transitional, and dementia care needs in the heart of downtown St. Paul. As part of Fairview's Community Health and Wellness Hub, EICR is committed to delivering innovative, collaborative care. The Fairview Community Health and Wellness Hub opened in 2022 to deliver a variety of services to the community—from primary and mental health care to enrichment options for seniors, food access programs, and community gathering spaces. The Hub brings Fairview services together with local organizations—including Minnesota Community Care and Sanneh Foundation—to make it easier for people to access what they need to be well.

We are seeking a Nursing Assistant (CNA/NAR) for our newest Skilled Nursing Community (Transitional Care units, Long Term Care) in downtown St. Paul, Minnesota! This role is responsible for providing resident care and services.

This CNA/NAR schedule includes:

• 48 hours per two weeks; Part Time
• Night shift
• Block Schedule: Week 1: Wednesday, Thursday Week 2: Monday, Friday, Saturday, Sunday
• Transitional Care Unit (TCU)
• Every other weekend and holiday rotation

Responsibilities

• Assists residents with personal cares based on individual plan of care:
  • Provides AM/PM cares
  • Provides full bath/shower weekly
  • Provides oral hygiene per care plan
  • Provides nail care, shaving as appropriate
  • Checks hearing aids; cleans eyeglasses daily
  • Assures that residents are clean, dry, and dressed appropriately
  • Provides offloading for residents
  • Performs Restorative Nursing Programs per facility policy.
  • Provides adequate nutrition & hydration (distributes meal trays, assists residents as needed, documents intake/output meal consumption, provides nourishment & water between meals as ordered)
  • Assures individual toileting plan is followed
  • Answers call lights promptly
• Observes residents and reports status to appropriate staff:
  • Receives report at the start of the shift
  • Gives report to team leader at the end of the shift & PRN
  • Notifies appropriate staff of any changes in a timely & appropriate manner (i.e. incidents, skin integrity, behavior changes, illness)
  • Offers pertinent observations & input for documentation and care plan updates
  • Utilizes Nursing Assistant Kardex or Care sheet
  • Takes and documents weights as ordered
  • Documents resident cares promptly in EHR
  • Responds to emergency situations appropriately
  • Handles resident and family complaints tactfully and refers concerns to appropriate staff members
  • Follows Ebenezer policies and procedures
  • Reports resident change of condition to Licensed Nurse
  • Attends mandatory, annual, and any other educational opportunities on site.
• Performs environmental and other responsibilities as assigned:
  • Maintains order & cleanliness in resident rooms
  • Maintains order & cleanliness in other work areas
  • Reports malfunctioning equipment to Maintenance Department
  • Assists with admissions, transfers & discharges as needed
  • Answers phones with proper customer service etiquette
  • Performs other duties as assigned and directed

Required Qualifications

• less than 1 year experience
• Nursing Assistant Registered on the MDH Nursing Assistant Registry by start date – OR- Individual who is hired into Ebenezer’s Nursing Assistant Course or Ebenezer’s Pre-CNA job for Nursing Assistants; will be granted a 4-month post hire to become active on the Minnesota Nursing Assistant Registry.
