Job summary

Ebenezer is seeking a Registered Nurse (RN) to support residents at the Pillars of Prospect Park in Minneapolis, ensuring safety and health needs are met, conducting assessments, coordinating services and maintaining regulatory documentation.

The role includes casual/on-call shifts with day or evening options and a requirement to pick up three shifts per month, including two weekend shifts. MN RN licensure and BLS are essential, with supervisory responsibilities and staff support.

Qualifications

  • Vocational/Technical Training or Associate Degree
  • RN license in Minnesota (registered with MN Board of Nursing)
  • Current BLS certification (AHA/Red Cross) and active enrollment if needed on hire

Responsibilities

  • Maintain resident health status with ongoing assessments per Minnesota regulations.
  • Coordinate internal/external health services and ensure physician orders are implemented.
  • Oversee medication administration and ensure documentation accuracy.
  • Supervise and train staff; participate in scheduling and performance monitoring.
  • Ensure compliance with Federal, State and Local laws and report to DHS/Executive Director as required.
  • Respond to health events, collaborate with care teams, and support emergency planning.

Job Overview

Opened in 2020, the Pillars of Prospect Park is conveniently located off the light rail transit station and close to the metro bus near the University of Minnesota campus in Minneapolis, MN. This senior living community features 133 Independent living, 106 assisted living, and 44 memory care apartments. Two buildings make up this vibrant community and also encompass an on-site Intergenerational Child Care Center. Managed by Ebenezer, Minnesota's largest senior living operator. The Registered Nurse (RN) is responsible for ensuring that the safety and health needs of the residents are met.

This Nursing schedule includes;

  • Casual/On Call
  • Day or Evening shifts
  • Must pick up three shifts a month; two being weekend shifts
Responsibilities
  • Maintains current assessment and documentation of resident health status:
    • Performs initial and on-going assessments per the applicable Minnesota Assisted Living regulations and Nurse Practice Act. Ensures assessments are completed every 90 days per regulatory requirements.
    • Works with resident and/or responsible party through collaboration, education, and timely communication to ensure appropriate service and placement.
    • Service Agreements: assure that resident has an up to date service agreement at time of admission, with any changes in services and annually.
    • Develops, maintains, and implements individual emergency plans based on health status and Advance Care Planning documents, including POLST.
    • Maintains clinical charts and progress notes using computerized documentation system.
    • Communicates changes in resident conditions to medical and other relevant individuals and perform the appropriate follow up to health needs
  • Coordinates resident health services:
    • Facilitates coordination of services with internal and external providers.
    • Assures that all physician orders are signed and implemented per the applicable Minnesota Assisted Living regulations.
    • Works with resident and/or representative(s) to assure delivery of appropriate services and funding.
  • Manages and oversees medication and treatment administration:
    • Assures medications and supplies are ordered in a timely manner.
    • Assures medication and treatment practitioner orders are renewed at least annually and as necessary when there is change in medication and/or treatment. Minnesota Assisted Living regulations will be followed in obtaining and implementing practitioner orders.
    • Assures medi boxes, bubble packs, and/or syringes are filled per medication orders/changes for medication administration by designated staff.
    • Assures that all medication administration procedures are correctly followed. Takes appropriate action with staff regarding any problems or issues.
    • Assures accuracy of medication and treatment records, including documentation of administration and appropriate signatures.
  • Monitors work and performance of licensed and unlicensed staff:
    • Meets with staff to coordinate care and address care issues.
    • Plans and assigns/delegates work to staff within established guidelines.
    • Interviews, trains, and orients new employees as directed.
    • Assists and supports staff to complete assignments as needed.
    • Assists Director of Health Services (DHS) in monitoring and evaluation of staff performance.
    • Assists DHS in scheduling activities that support efficient and effective utilization of staff as needed.
  • Schedules and completes Supervisory Visits.
  • Complies with Federal, State and Local Laws.
  • Support DHS to act as RN on call during scheduled times.
  • Answers calls and gathers information related to assisted living community residents health and safety.
  • Work collaboratively with indentified internal and external support providers.
  • Actions taken may require phone directives and/or traveling to the assisted living community to resolve the situation.
  • Reports to the DHS and/ or Executive Director if further follow up is needed
  • Performs any other duties as delegated by Director of Health Services.
Required Qualifications
  • Vocational/Technical Training or Associate Degree
  • less than 1 year experience
  • Proficient in Microsoft Windows 7
  • Registered Nurse RN Licensure/Registered with the MN Board of Nursing
  • Basic Life Support (American Heart Assoc or Red Cross) Current Basic Life Support (BLS) for Health Care Provider from American Heart Association (AHA) or American Red Cross; accepted certifications as follows -AHA: BLS for Healthcare Providers (CPR & AED) Program or BLS Provider - BLS Instructor. American Red Cross: CPR/AED for Professional Rescuers and/or Healthcare Providers, Life Guarding First Aid/CPR/AED. BLS is required upon hire and/or new hire must show proof that they are currently enrolled in a BLS class to renew or obtain their certification. BLS certification must be obtained within three-weeks of hire date and before nurse can perform independent practice.
Preferred Qualifications
  • Bachelor of Science Nursing
  • One to Three Years experience
  • Proficient in Eldermark Computer Documentation Software
  • Prior supervisory experience.
  • Prior experience in home care or assisted living environment.
