Job summary

Belmont Village Senior Living seeks a Wellness Coordinator to manage clinical and care aspects of the community and support regulatory compliance and quality monitoring. A licensed nurse (LVN/LPN/RN) is required, with strong collaboration with practitioners, families, and staff to ensure timely clinical interventions and follow-up.

You will lead Caregivers and Medication Technicians, oversee documentation, and uphold infection control.

Qualifications

• Active LVN or LPN nursing license in the state where the community is located.
• CPR certified or ability to obtain within 30 days of hire.
• Minimum one year of medication administration experience preferred.
• Prior supervisory experience preferred.
• Clear verbal and written English communication required.
• Intermediate computer and mobile device skills.
• Professional, pleasant and team oriented attitude.
• Consistent and reliable attendance is essential for Belmont Village employees.

Responsibilities

• Coordinate day-to-day operations of the Wellness Center.
• Follow up on physician orders, coordinate services with providers, and report changes to physicians/families.
• Administer medication to residents per physician orders and document observations.
• Ensure care plans are followed by staff.
• Make situational decisions and manage conflicts constructively.
• Update resident medical records and incident reports.
• Assist in regulatory compliance and enhancing resident satisfaction.
• Assist in infection control and hazard communication programs.
