Belmont Village Senior Living is seeking a Wellness Coordinator in Illinois to oversee clinical and care aspects of the community, in collaboration with the Director of Nursing. You will lead Caregivers and Medication Technicians, ensuring timely interventions and quality monitoring for residents.

The ideal candidate is reliable, empathetic, and thrives in a fast-paced, active setting with strong teamwork and hospitality for seniors.

Qualifications

  • Active Illinois nursing license (LVN/LPN or RN).
  • CPR certification or ability to obtain within 30 days.
  • Minimum one year of medication administration experience.
  • Prior supervisory experience preferred.
  • Clear verbal and written English communication.
  • Intermediate computer and mobile device skills.
  • Reliable attendance essential for Belmont Village.

Responsibilities

  • Coordinate day-to-day operations of the Wellness Center.
  • Follow up on physician orders and coordinate services with providers.
  • Administer medication per physician orders and document observations.
  • Ensure care/service plans are followed by staff.
  • Make situational decisions and resolve conflicts constructively.
  • Update resident medical records and incident reports.
  • Support regulatory compliance and resident satisfaction; assist in infection control training.

ABOUT THE ROLE

As a Wellness Coordinator at Belmont Village, you will oversee and manage clinical and care aspects of the community while supporting the Director of Nursing in wellness initiatives, regulatory compliance and quality monitoring. You must be a licensed nurse – LVN, LPN or RN. You will communicate with healthcare practitioners, ancillary providers, pharmacies, family members, and team members as needed ensuring timely clinical interventions and follow-up. You will lead and provide direction to Caregivers and Medication Technicians where applicable. The ideal candidate is reliable, takes pride in their work, and demonstrates empathy. They excel in a fast-paced, physically active position, collaborate effectively, and embrace a growth mindset, prioritizing exceptional hospitality and helping seniors to thrive as they age.

YOUR TYPICAL RESPONSIBILITIES
  • Coordinate day-to-day operations of the community Wellness Center
  • Follow up on physicians orders, coordinate services with ancillary providers and report to physicians and families any changes of condition or other pertinent clinical care information
  • Administer medication to an assigned group of residents according to physicians orders and report any adverse reactions or clinical observations documenting as necessary
  • Ensure resident care and service plans are followed by supervising Medication Technicians and Caregivers
  • Make appropriate situational decisions and manage conflict resolution constructively
  • Update and review resident medical records, clinical documentation, and incident reports
  • Assist the Director of Resident Care Services in regulatory compliance and enhancing resident satisfaction
  • Assist in maintaining and training of infection control and hazard communication programs
MINIMUM QUALIFICATIONS
  • Active LVN or LPN nursing license in the State in which this community is located
  • CPR Certified or ability and willingness to obtain within 30 days of hire
  • Minimum one year of medication administration experience preferred
  • Prior supervisory experience preferred
  • Must be able to communicate clearly in verbal and written English
  • Intermediate computer and mobile device skills
  • Professional, pleasant and team oriented attitude
  • Consistent and reliable attendance is essential for all Belmont Village employees to ensure the successful operation of the community
