Premier Rehab & Healthcare at Burlington is seeking an experienced Unit Manager – Long-Term Care (LTS) to join our nursing leadership team. The role oversees the Long-Term Rehabilitation unit, supervising nursing staff and coordinating interdisciplinary services to deliver resident-centered care.

The ideal candidate will have strong clinical and leadership experience in rehabilitation or long-term care, with a Vermont RN license and a commitment to quality outcomes.

Qualifications

• Current, active Vermont RN license.
• Leadership or supervisory experience preferred.
• Experience in rehabilitation, long-term care, or subacute care preferred.
• Strong clinical assessment and critical-thinking skills.
• Knowledge of long-term care regulations.

Responsibilities

• Oversee daily operations of the Long-Term Rehabilitation unit.
• Supervise nursing staff to ensure high-quality resident care.
• Collaborate with physicians, therapists and other team members.
• Coordinate and implement individualized resident care plans.
• Monitor resident assessments, treatments and outcomes.
• Ensure compliance with state and federal regulations.
• Lead admissions, discharges, and care transitions.
• Provide leadership, mentoring and support to staff.
• Participate in quality improvement initiatives.
