Job summary

Huntington Health & Rehab Center in Huntington, West Virginia is seeking a Nurse Supervisor (RN preferred; LPN with experience considered) to lead nursing care on our skilled nursing unit, ensuring quality resident care and adherence to policies.

You will plan and coordinate interventions, administer medications, mentor staff, and collaborate with therapy teams to support rehabilitation goals. We offer daily pay options, competitive shift differentials, and a comprehensive benefits package.

Qualifications

• Active state RN license
• CPR/BLS Certification
• Experience in Long Term Care or Skilled Nursing preferred
• Excellent technical, assessment and documentation skills
• Strong analytical and problem-solving skills
• Ability to lift 25+ pounds
• Ability to stand, bend, and walk for extended periods
• Team player and strong communicator

Responsibilities

• Provide nursing care to ensure resident needs are met per standards and physician orders
• Plan nursing interventions for each resident and monitor implementation
• Evaluate care interventions and identify problems/trends
• Conduct family and resident education
• Administer medications and treatments timely
• Transcribe physician orders and document progress
• Identify and report changes in condition to supervisor
• Coordinate rehabilitation program with therapists
• Ensure CNA documentation and assignments are completed on time
• Perform additional nursing tasks as required by state Nurse Practice Act

What You’ll Be Doing:

• Provide nursing care to ensure resident needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines
• Plan nursing interventions for each resident based on individual needs and monitor the implementation
• Evaluate the effectiveness of care interventions, identify problems and trends and develop alternative interventions.
• Conduct family and resident education
• Administer medications and treatments timely and according to center policy
• Transcribe physician’s orders and chart progress notes in an informative, factual manner that reflects the care administered as well as the resident’s response
• Identify and report changes in condition to supervisor, physician and family
• Accurately identify skin changes and follow skin management protocols
• Order and report diagnostic tests timely
• Follow Pharmacy policy and procedures for ordering and delivering medications.
• Coordinate resident’s rehabilitation program with therapists to ensure rehabilitation needs are met
• Monitors that CNA documentation is completed daily
• Ensures CNA assignments are completed on time
• Perform additional nursing care responsibilities as required and based on State Nurse Practice Act.

Requirements:
