Esperta Health is advancing in-home wound care through evidence-based practices, led by a Nurse Practitioner - Wound Care Team Leader. You will supervise wound care nurses, coordinate with interdisciplinary teams, and drive clinical outcomes in the assigned market.

The role emphasizes leadership, patient-centered care, and adherence to regulatory standards, with a focus on continuous quality improvement and staff development.

Qualifications

• Current, unrestricted NP licensure in state(s) of practice.
• WOCN or equivalent wound care certification preferred.
• Minimum 3–5 years of wound care clinical experience, preferably in home or post-acute settings.
• Demonstrated leadership experience in clinical settings or wound care programs.
• BLS certification or higher.
• Proficiency with electronic health records and digital wound care management tools.
• Excellent communication, organization, and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Valid driver's license and reliable transportation for in-home visits.
• Strong commitment to patient-centered care and quality improvement.

Responsibilities

• Provide expert wound care assessments, treatment planning, and management in home settings.
• Lead and mentor wound care nurses within the assigned market.
• Collaborate with physicians, case managers, and other healthcare professionals.
• Monitor clinical quality, compliance, and performance metrics.
• Serve as a clinical resource and subject matter expert for wound care.
• Participate in quality improvement activities and protocol development.
• Engage with patients and caregivers to provide education and support.
• Demonstrate leadership to foster teamwork and clinical excellence.
